How we work

Most practices list the conditions they treat. The more useful question is how the work is actually done, because the method matters more than the label on your difficulty.

Internal Family Systems

IFS is the model we are best known for. Rather than treating a symptom as something to remove, it treats your reactions as parts of you that once made sense and are still trying to help. The work is learning to hear them without being run by them.

Keith Miller is a Certified Level III IFS therapist with advanced IFIO training, the IFS approach to couples, and teaches it nationally. Read more about IFS.

Ketamine-assisted psychotherapy

Keith founded Calliope Health, a ketamine-assisted therapy clinic based here in Bethesda. It pairs a medical protocol with prepared, supported therapy sessions rather than treating the medicine as the treatment on its own, and is used where depression, anxiety or trauma have not responded to talking therapy alone. How it works.

Mindfulness-based therapy

Our usual starting point for anxiety is mindfulness-based psychotherapy rather than medication. The aim is deliberate access to your parasympathetic nervous system, the brake on your body’s stress response. Reaching it on purpose is a learnable skill, and it often changes how manageable anxiety feels early on.

Relationship therapy

Our clinicians draw on Imago Relationship Therapy, the Gottman Method and Emotionally Focused approaches, and Keith’s IFIO training brings IFS into couples work. Relationship work has its own page — see marriage and couples counseling in Bethesda.

Attachment patterns

A great deal of what brings people to therapy is not a diagnosis but a pattern: pulling away when someone gets close, or needing constant reassurance and hating that you need it. These are attachment styles, and they are learnable.

Avoidant patterns often look like independence from the outside. Closeness registers as pressure, so you create distance and then feel the loneliness of it. Anxious patterns run the other way — the fear of being left produces exactly the behaviour that pushes people away.

Both respond well to therapy, and IFS suits this work because it treats the protective behaviour as something with a reason rather than a flaw.

Who we help

Anxiety and stress

Panic, phobias, and worry that has started to shrink your life. This is one of the most common reasons people contact us.

Depression

Depression is often treated with medication alone, without much attention to what is driving it. Combining medication with talk therapy generally works better than either on its own.

Trauma and PTSD

Trauma work needs a method built for it. We use IFS and, where appropriate, ketamine-assisted psychotherapy when talking therapy alone has not shifted it.

Anger

Anger is not confined to people whose temper gets them into obvious trouble. It shows up quietly too, as resentment finding passive routes out inside a relationship.

Addiction

Whether the concern is alcohol, drugs, food or sex, the pattern runs through emotion rather than around it. Understanding how emotions feed the cycle is what makes replacement behaviours hold up.

Postpartum depression

New parenthood can arrive with feelings nobody prepared you for. It is common, treatable, and not a verdict on you as a parent.

Bipolar disorder

Mood regulation work combining therapy with psychiatric care, both available from this office.

Grief

Grief has no schedule and does not follow the stages it is supposed to. Support for loss, including losses others expect you to be over.

Psychological testing

IQ, ADHD and educational testing for children and adults across Maryland.

Psychiatry and psychological testing in Bethesda

Two services run from this office that most local practices do not offer at all. Anna Richards, a board-certified Psychiatric Mental Health Nurse Practitioner, provides psychiatric evaluation and medication management here for $250. Having psychiatry and therapy under one roof means the two can actually coordinate rather than working past each other.

An affirming practice

Members of the LGBTQ+ community are welcome here. We work with questions of sexuality and gender identity as part of a life, not as the problem to be solved. We also work with people in consensually non-monogamous and polyamorous relationships, who deserve a therapist who is not spending the session catching up on the structure.

Areas we serve near Bethesda

The Bethesda office serves clients across Montgomery County and the wider Maryland suburbs. People come to us from North Bethesda, Chevy Chase, Rockville, Silver Spring, Potomac, Gaithersburg, Wheaton, Takoma Park and Glen Echo. Our second office is in Dupont Circle, Washington DC.

Fees and insurance

Fees at the Bethesda office are $250 to $450 for a 50 minute session, depending on the clinician you see. Psychiatric evaluation with Anna Richards is $250. Payment is taken by card before your first session.

We do not bill insurance directly. Our clinicians are licensed, so our services qualify for reimbursement under most plans, and clients who submit their own out-of-network claims commonly recover 40–60% of what they pay. We provide the statement you need. See our full policies and fees.

Frequently asked questions

Do you offer online sessions?

Yes. You can meet your counselor in person at either our Bethesda or Washington DC office, online by video, or by phone. Many clients alternate between them.

Do you accept insurance?

We do not bill insurance directly, but many clients submit their own out-of-network claims and often receive 40–60% of the expense back. One practical note for couples: most insurers will not reimburse against “marriage counseling” as a description. The reimbursable procedure code is conjoint psychotherapy, code 90847.

How do I choose a therapist?

You do not have to. Call the referral coordinator on extension 0, describe what you are dealing with and when you can meet, and we will match you with the clinician best suited to it, not whoever happens to be free.

How quickly can I be seen?

We offer a free phone consultation before you commit to anything. Weekday appointments before 4pm have the widest choice of clinicians; evenings and weekends fill faster.
